Change of Transportation Form
Student safety is our top priority. All transportation changes must be submitted in writing by parents.
Step 1
Submit changes in writing via parent/guardian by scanning the QR code or using the detachable section turned into the front office.
Step 2
All transportation change requests must be received by 2:00 PM on the same day.
Step 3
This ensures sufficient time to communicate changes effectively and keep your child safe.
- Ms. Gonzalez and Ms. Caesar will communicate transportation changes over the classroom intercom.
- Parents will need to complete the form by 2:00 PM. After 2:00 PM, no additional changes will be accepted for the day.
- If parents call the front office, the QR code will be sent via email. All transportation changes MUST BE IN WRITING!
- Submission Options: Scan the QR code or detach the bottom section of this form and drop it into the front office mailbox (GOLD MAILBOX).
- Early Dismissal ends at 2:30 PM sharp unless there is an emergency.
